“Continuum of Care” is a concept which illustrates an integrated system of health care that guides and tracks patients through various health services spanning all levels and intensity of care. The continuum begins with the patient and includes the entire network of health care professionals and caregivers that directly or indirectly care for any given patient.
Stakeholders along the continuum might include, but are not limited to, physicians, insurers/payers, home-health agencies, pharmacies and family members/friends. The care continuum plays a critical role for the patient, provider(s) and payer(s), treating each as interdependent components of a much larger system.
