Alana Healthcare’s Chronic Conditions Outcome Management (CROM) program focuses on patient-centered care. Our healthcare professionals work with patients across the entire chronic disease spectrum. Payers and providers partner with Alana Healthcare to offer their patients a wide range of education and services to help them manage their chronic diseases.
Home Based Therapy (HBT)
Alana’s new Home Based Therapy (HBT) program supports patients transitioning from hospitals back into the comfort of their own homes. Our HBT program provides invasive ventilation and noninvasive positive pressure ventilation (NIPPV) in conjunction with our full spectrum care program, Comprehensive Respiratory Outcome Management (CROM).
