Alana’s program directors and patient care teams work with you to develop a plan and process that best fits your needs and your chronic care management situation. Our highly specialized team delivers comprehensive healthcare monitoring programs aimed at providing you with the resources you need to avoid hospitalizations.  

You will receive education on nutrition, exercise, medication management, good health habits, self-managing your own health, and improving your quality of life. Once enrolled in our program, you will have access to the Alana Healthcare care team 24/7/365.

Programs:

Alana Healthcare programs provide in-home visits by licensed clinicians to evaluate health markers/vitals and provide disease-specific education. Our programs also include telephonic education, Nursing triage, and access to our Social Work team. 

• CROM:  Chronic Respiratory Outcome Management: This is a 6-12 month program designed to help individuals diagnosed with chronic respiratory disease to better-understand and maintain their lung health.

• HSP: Heart Success Program: This is a 6-12 month program designed to help individuals diagnosed with chronic Cardiac disease to better-understand and maintain their heart health.
